This presentation will provide two perspectives on transition planning from Tamarack Center in Spokane, Washington and from Trillium Family Services in Corvallis, Oregon. From Tamarack Center, we will learn about best practices in individualized discharge planning for youth in residential care. The presenter will discuss how to use social leave days and community resources to provide the most effective discharge planning into the community and the family setting. From Trillium Family Services, we will learn about the evidence-based practice of Collaborative Problem Solving and its implementation in a residential unit on its Corvallis campus. Attendees will learn about training of staff and how the team built a network of community partners and families.
